I'm honored you picked me to create something so special for you! This is a form to make the booking process more efficient.

**I am only accepting small/medium size tattoos right now. I am not booking coverups or large body pieces. Sorry for any inconvenience.**

First, tell me what you’re wanting tattooed, the size and location, and if it will be colored or not. Let me know any other details you think are necessary.

Second, after I've reviewed the request, I will contact you and go more in depth on the design.

​Third, I will send you an invoice detailing the agreement. There will be a non refundable deposit of $50 that goes towards your tattoo. The remaining balance can be sent once it is completed.
